The team provides care and support to help adults who have complex or chronic diseases, stroke, brain injury, a fall, and who need extra help to live independently.

The team works with patients and their relatives or carers. The aim is to promote health and independence in mobility, communication, work, leisure and everyday activities. The team often helps patients through the transition as they return home from hospital. They can also provide support to help avoid people being admitted to hospital for practical rather than medical reasons. The team is made up of many healthcare professionals including physiotherapists, occupational therapists, speech and language therapists, doctors and nurses.
