The service is for adults with neurological disorders and / or a brain injury requiring rehabilitation towards independence or semi-independence within the community. A comprehensive initial assessment is followed by the development of a personalised care and support plan to meet the service user's personal and functional goals.

An expert rehabilitation team will work with the service user on their individual programme, utilising external resources where appropriate, to achieve specific outcomes over the short, medium or longer term.

When progress has been achieved, Consensa can provide a managed pathway to more independent accommodation.

Typical duration that a service is offered

There is no set limit to the duration of service on offer. Consensa's service is completely tailored to the needs of the individual service users. We are able to offer short-term respite care where this need is identified, or alternatively, where there are complex issues requiring a long term rehabilitation plan, Consensa is able to offer care for a number of years. Typically we would expect to 're-able' and optimise an individual's functional ability within 6 months to 2 years.
